Application for Use of School Facilities
Instructions on filling out Application:
Click on the Application link above. You will see a gray box that asks you if
you would like to open the file or save it. Click Open. An OmniForm Mailable
Filler Box will come up. Click OK. The Use of School Facilities Application
will then appear. This application is intended to be filled out on your computer,
printed and mailed or brought to the following address:

When you close the application box you will be asked if you want to save changes. Click No.
